Medication Chart

I recommend using a medication chart any time you need to give a medication to a pet. They do not take long to fill out and when you follow all of the steps of filling out a medication chart, you are much less likely to make a mistake

You can download an example medication chart by following the link in the description below.

Filling Out The Medication Chart

Now that you know how serious medications can be, let’s cover an example of filling out the medication chart.

Again, review all of the information you have about the pet and the medication.

  • Pet’s Name
  • Pet Description and markings
  • Medication Name(s)
  • Medication’s instructions on the bottle
  • Look for any hand written notes the client may have left out with updates about the medication
  • Read the notes in your online system.

Once you fully understand the medication instructions and you are sure you are administering the correct medication to the correct pet at the correct time – fill out each section in a clear, detailed manner. Write the instructions in a way that leaves no room for confusion or doubt about how to administer the medication.

I also recommend waiting until AFTER you give the medication to mark down the time and your initials. This way if you get sidetracked by something at a visit, you don’t have to think about if you actually gave the medication or just filled out the sheet in advance.
